2025 Community Health and Needs Assessment Executive Summary
2020 Community Health and Needs Assessment: Every five years, the Florida Deparment of Health in Charlotte County comppletes the Community Health and Needs Assessment (CHNA), which is then used to produce and inform a Community Health Improvement Plan (CHIP). The purpose of the CHA is to review and analyze relevant data to provide invaluable information on the overall health and well-being of our community. However, the 2019-2020 CHA is composed of a much robust analysis that considered the economic, social, environmental, behavioral, and educational factors, which all invariably impact individual and population level health outcomes.
2001-2025 Community Health Improvement Plan (2023 revision): The Florida Department of Health in Charlotte County presented this CHIP as part of its commitment to the continued assessment and development of community-based interventions to improve the health and well-being of all residents. The plan was developed based on the 2020 CHNA completed in February 2020, in which residents identified strategic areas of concern within the community.
